Make a referral

You can make a referral to Services for Young People if you are a young person (self-referral), a parent/carer or a professional who works with young people.

Services for Young People (SfYP) provides targeted prevention and early intervention youth work, information, advice, guidance and work-related learning for young people who live or attend school in Hertfordshire and are aged 11 (Year 7) to 17 years old. We also work with care leavers, young adults with learning disabilities (LD) and some other vulnerable groups up to the age of 25.

If the presenting need(s) of the young person are unlikely to be met through prevention and early intervention, please contact the Customer Service Centre on 0300 123 4043 for advice.

How do I make a referral?

You should make a referral by filling in the form below. Alternatively, you can download and complete a Word version of the referral form and email it to [email protected].

What happens when I make a referral?

When you make a referral, it will be sent to the relevant Services for Young People team and you will be contacted within a few days to discuss how best we can meet the young person's needs.
